Identify the tree on photos and a plan, then explain the reason: changing condition, observed damage or a development proposal. Being close to a house does not automatically require removal. Have the situation and alternatives assessed first. State whether you also expect stump removal and ground reinstatement.
Confirm the applicable procedure

Document the decision and scope
Request assessment suited to the issue and identify the tree unambiguously in the quote. Provide existing decisions or conditions and allocate responsibility for missing documents and scheduling. A landscaping constraint alone is not permission to fell.
Have the method determined on site
Identify buildings, roads, utilities, retained trees, access and handling space. Ask the contractor to define method and site protection after assessment. An obstacle alone does not prescribe climbing or rope dismantling. Coordinate work near utilities with the operator beforehand.
Plan the ground afterwards
Specify retained wood, chipped or removed branches and storage location. State whether the stump remains, is ground or is extracted as additional work. Define planned or required planting separately. Removal of a tree alone does not make the ground immediately ready for building.
